Terms of Service
These Terms of Service set out the basis on which Surrey Quays Dental Practice provides care and treatment to our patients. By booking an appointment and receiving treatment at the practice, you agree to the following terms:
1. Payment and Finance
All treatment fees must be paid at the time of treatment, unless otherwise agreed as part of a written treatment plan.
Where finance is required, this must be arranged before treatment begins.
If finance is not in place, patients are required to make full payment at the time of treatment or in accordance with the agreed treatment plan.
Failure to make payments according to the agreed schedule may result in delayed or cancelled appointments.
2. Deposits and Cancellations
A deposit of 20% of the treatment cost or the cost of materials (whichever is higher) will be taken at the time of booking.
- This may reduced in the case of a longer treatment plan.
Deposits are non-refundable if less than 2 working days cancelation notice is given.
Deposits will be forfeited if an appointment is missed without adequate justification and supporting evidence.
3. Treatment Plans and Quotes
Any treatment plan or payment plan provided by the practice is given as a quotation only and is subject to change.
Costs may fluctuate due to factors beyond our control, such as laboratory fees, material costs, or changes in clinical requirements.
A final invoice will be issued at the completion of your treatment, reflecting the actual costs incurred.
The practice will make every reasonable effort to notify you in advance of any significant changes to your treatment costs.
4. Emergency Treatment
Emergency treatments may attract an additional charge at the discretion of the treating dentist.
Any such charges will be clearly explained and agreed during the emergency consultation before treatment proceeds.
5. Treatment Responsibility
Each dentist at Surrey Quays Dental Practice is an independent associate, responsible for the care and treatment they provide.
Complaints relating to treatment must be directed to the treating dentist.
6. Treatment Outcomes and Refunds
All dental treatments carry an inherent risk of failure, which will be explained to you during your consultation.
Refunds will only be issued where treatment failure is directly attributable to a member of the dental team.
Refunds will not be given if treatment fails due to:
Failure to follow instructions from the dental team
Failure to use prescribed or recommended products correctly
If a product used in your treatment fails, you may be required to seek redress directly from the manufacturer.
7. Legal Action for Non-Payment
Failure to pay treatment fees in full, or according to the agreed treatment plan, may result in treatment being delayed or cancelled.
Where payment remains outstanding, the practice reserves the right to pursue the debt through legal proceedings.
8. Patient Responsibilities
By attending appointments at Surrey Quays Dental Practice, patients agree to:
Provide accurate medical history and personal details.
Follow professional advice and treatment instructions provided by the dental team.
Treat staff and other patients with courtesy and respect.
9. Missed NHS Appointments
If you are an NHS patient who misses two appointments without reasonable explanation or advance notice, you will removed from the register of NHS patients within Surrey Quays Dental Practice. Any payment made in advance of treatment may be retained to cover costs such as purchased materials, and payment of time for the associates and employees of Surrey Quays Dental Practice.
10. Data Protection
All patient information is stored and processed in line with our Privacy Policy, which complies with the UK GDPR and the Data Protection Act 2018.
11. Recording in the practice
Surrey Quays Dental Practice utilises security cameras within public areas of the practice. Two types of cameras are in use. Ones that are activated when required and linked to the alarm system, and an always on audio and visual recording system. By entering the practice you agree to be recorded in this environment to keep our staff safe.
If you wish to take any pictures in the practice, please ask permission to do so. This is a private business and does not give you an automatic right to record video, audio or take pictures.
We may ask to take pictures of your treatment for records, CPD, or for advertisement of the business or the individual member of staff or associate. This will be made clear to you at the time, and consent obtained in writing. This does not affect your rights under the relevant legislation.
